| 医疗服务会通过影响农村老人的健康水平而对其失能状况产生影响。基于2011—2012年全国老年健康影响因素调查数据（CLHLS），将农村老人失能状况分解为自理能力丧失、生活能力受限以及认知能力障碍，利用二元Logit模型实证研究了医疗服务对农村老人失能状况的影响，并对相应的影响机制进行了探讨。研究发现：是否参加新农合、距医疗点距离、医疗费用分担方式、生病能否得到及时救治以及社区医疗服务会显著影响农村老人失能状况的发生。总体上，样本地区农村老人失能状况较为严峻，并呈现出明显的性别差异和年龄差异。建立完善的长期护理体系并通过优化医疗服务供给来增强农村老人对医疗服务的可得性与可及性，成为解决农村老年失能问题的政策选择。 |

| Public medical services can affect the health status of the elderly in rural areas and then have impact on the health of the elderly. Based on the survey data of Chinses Longitudinal Healthy Longevity Survey of 2011, this paper decomposed the rural elderly disability into self-care ability loss, limited capacity of life and cognitive impairment, by using the two element Logit model studied the effect of accessibility and accessibility of medical services on the disability status of the elderly in rural areas, And discussed the corresponding influence mechanism. The research outcomes are as follows: New Rural Cooperative Medical participation, distance of residents go to the nearest medical organization, medical service compensation method, the disease can be treated in a timely manner and community health services will significantly affect the occurrence of rural elderly disability. On the whole, rural elderly disability is more severe in the sample area and showed obvious gender difference and age difference. Establish a long-term care system and optimize the supply of medical services to enhance the availability and accessibility of medical services to farmers is becoming a policy choice to solve the problem of rural elderly disability. |
